The employee benefits survey template below is short and simple with four total questions. A matrix question asking to rate current benefits, a MaxDiff (explained below) and ranking question to help you identify what benefits are most important, and an open ended question asking employees to recommend new benefit offerings.
Asking multiple choice questions on your benefits survey will not identify what employees truly value. To get a more accurate assessment, a MaxDiff or ranking question should be used. Both question types are unique to SurveyKing and are included in the survey template. Below is a quick explanation of how they can add valuable data to your survey.
This question type ask respondents to pick what is least and most important from a set of attributes, or in this case benefits. Data from this question can be used to shape new benefit offerings and even find cost savings. You might notice PTO is more important than a lower health care deductible. This is actionable data that can be used in decision making.
This question type asks respondents to rank in order a certain number of attributes. In this example the respondents can pick their top two benefits.
The anonymous survey link is perfect to collect feedback for an employee benefits survey. Employee location could be included as custom data, and results could then be segmented by location. You may notice one region has different needs than another region. When using an anonymous survey link, identities will be protected using k-anonymity, even when custom data is included.
The anonymous email collector would be another method of anonymous data collection. This collector type gives you the ability to upload custom data directly from an Excel file and send reminders to employees who have not yet taken the benefits survey.
A web link is like the anonymous link except that you can create multiple web links per survey, and identities are not protected. For companies with multiple departments, a different web link for each department could be used. Results could then be segmented by collector, and different benefits based on department can be identified.
Text messages are a great way to improve response rates for an employee benefits survey. You can upload a list of employees, and our system will automatically send a text message to each. Reminders can be sent to employees who have not responded.
Email Invitations also provide another way to quickly send invites to a list of employees. Our email platform will include open rates, click rates, and bounce rates. Reminders can be sent to employees who have not yet completed their survey.

The ranking question will compute a weighted calculation score. Benefits ranked first are given a higher value or "weight." The score, computed for each benefit, is the sum of all the weighted values. The data table will also include a distribution of how each benefit was ranked.
